Our Process for Massachusetts Patients

Step-by-Step Guide for New Patients

Complete and Email New Patient Paperwork: Fill out the required new patient forms, including a copy of your ID and email them to us [link]. No medical records are needed.

Schedule a Telemedicine Appointment: Book your appointment online [link]. You’ll receive a confirmation email. Please note that we will call you at your scheduled time. FYI, the Telemedicine Appt is just over the phone.
Obtain Your MMJ Certification: During your telemedicine appointment, you’ll receive your MMJ certification and a two-week temporary card right away.
Receive Registration Instructions: After your appointment, you’ll get an email from the Cannabis Control Commission (CCC) with online registration instructions [link to patient portal] for your MMJ card, typically arriving within 1-2 weeks.
Paper Registration Option: If you’re unable to register online, you can request a paper registration by calling the MMJ Program at 833-869-6820.

Step-by-Step Guide for Existing Patients and New Recertification Patients

Schedule a Telemedicine Appointment: Visit our website to book your appointment. You will receive a confirmation email shortly after.
Review Required Forms: Prior to your appointment, please review the online HIPAA Notice and Release and Acknowledgment forms on our website [link]. No paperwork needs to be emailed.
Appointment: We will call you at your scheduled appointment time.
Obtain Your MMJ Recertification: During your telemedicine appointment, you will receive your MMJ recertification. If your registration is expired, a two-week temporary card may be available.
Renew Your MMJ Registration:Please renew your MMJ registration by the date indicated on your card using the patient portal [link]. You should receive your new MMJ card in the mail within 1-2 weeks.

Our Process for Rhode Island Patients

Step-by-Step Guide for New Patients

Follow steps 1 and 2 outlined for new patients above.

Visit the Rhode Island Department of Health (RI DPH) MMJ Program online, create your account, and complete your application. Please leave the
provider form blank.

We will call you at your scheduled appointment time.

During the telemedicine appointment, we will review your information, and the provider will email you your completed provider form.

You can then submit the provider form along with your Rhode Island MMJ application.

You should receive your Rhode Island MMJ card in the mail.

Step-by-Step Guide for Existing Patients and New Recertification Patients

Follow steps 1 and 2 outlined for recertification patients above. New RI Recertification patients please email a copy of your ID also.

Log into your Rhode Island MMJ Program account and access your renewal or an original application. Please leave the physician form blank.

We will call you at your scheduled appointment time.

During the telemedicine appointment, we will review your information, and the provider will email you your completed provider form.

Submit the provider form with your recertification application.

You should receive your Rhode Island MMJ card in the mail.
